Request Data Deletion

This page is the form for asking IM65 to delete information it holds about you. Deleting on request is IM65's own policy. HIPAA's right under 45 CFR 164.526 is a right to have health information amended, not deleted; the same form takes a correction too.

It asks who you are — a Medicare beneficiary, a family member, a doctor or an insurance agent — and what you want deleted: your directory listing, your account, a Scope of Appointment record, a doctor referral record, or "not sure". It needs your name and an email address or a phone number. A record number is optional; the Privacy Officer finds the record from your details, then deletes or redacts it and tells you what was done within thirty days.

Read the limit before you file. Some records cannot be removed outright, because retention is required of us: Scope of Appointment records must be kept for ten years under CMS rules, and audit logs for six years under HIPAA. Where that applies, your identifying details are redacted and the empty record structure is kept.

It neither quotes coverage nor cancels a policy — a carrier handles that directly.
