Medicaid Office Locator

This tool points you at your state's Medicaid program. It asks your location once — either by browser location permission or by a five-digit ZIP code — works out the state from that ZIP using a built-in lookup table, and then shows that state's information.

What appears: the state program's name, its hotline with tap-to-call, published hours, a county office locator (pre-filled with your ZIP where supported), a link to the state's online application, the official website, and a map search for nearby offices. Below that sit eligibility reference figures for your state, printed with the program year they come from — income and asset limits for a single person and a couple, any special state program, a Medicare Savings Programs table with each tier's limits, whether the state enrolls SSI recipients automatically, and a cost-of-living note. A fourteen-item document checklist lets you tick off paperwork on screen. A separate print button produces a paper copy of the office contacts and the eligibility figures — not the checklist.

To show your state's information it needs only a ZIP code or location permission — no account, no personal details. At the bottom sits an optional card offering a call back from a licensed advisor; that one does ask for your name, phone and ZIP.

What it will not do: it does not decide whether you qualify and does not submit an application. The figures are published guidelines; your state Medicaid office makes the official determination.
