Privacy Policy

This is the privacy policy for IM65, LLC — a written statement of what the site collects, how it is used, who it is shared with, and how long it is kept. It carries an effective date and a last-revised date.

It lists what you provide directly: account registration details, professional credentials such as an NPN and licence states, agency affiliation, quote inputs entered during quoting (including age, state and health information), messages you send, and an optional mailing address — the policy states a mailing address is never required to use any tool on this site. It also lists what is collected automatically: IP address, device and browser information, pages visited, and session activity logs kept for security and HIPAA compliance.

Other sections cover how the information is used, sharing (the policy says personal information is not sold; it may go to service providers, to law enforcement or regulators when legally required, or to a successor in a sale), security measures, retention — HIPAA audit logs for at least six years under 45 CFR 164.530(j) — cookies, and children's privacy.

It needs nothing from you to read. Exercising a right takes an email.

What it will not do: it is not the HIPAA Notice of Privacy Practices — that is a separate page it links to — it does not knowingly collect information from anyone under 18, and it states plainly that no system is completely secure.
